Do you know a local organization or MGB employee that is making a difference in the health of communities? Nominate them to receive an award and recognition of their work.


Overview 

The Kraft Center for Community Health at Mass General Brigham (MGB) is pleased to invite nominations for the Kraft Center Community Health Champions Award. The Community Health Champions award, made possible through the generous support of the Kraft Center and M&T Bank, will recognize Massachusetts and New Hampshire-based community organizations and an MGB employee whose dedication and commitment to community service have made a positive impact on local community health.

Through an internal MGB nomination process, up to five (5) awards of $2,500 will be awarded to community-based organizations and MGB employees. Successful nominations will demonstrate the organizations’ and employee’s impact and commitment to improving community health. The focus of the work must also align with at least one of MGB’s priority areas: cancer, cardiometabolic disease, maternal health or substance use disorder. Initiatives can also include organizations or individuals whose efforts are addressing access, quality of care or social risk factors that impact these health outcomes.

Eligibility

Applications will be accepted through an internal nomination process. Any Mass General Brigham employee can nominate community-based organizations and MGB employees that meet the following criteria:

  1. Community-based organizations located in Massachusetts and New Hampshire that:
    • Focus on one more of the following priority areas: Cardiometabolic Disease, Substance Use Disorder, Cancer or Maternal Health
    • Access and quality of care or social risk factors such as food insecurity, economic mobility or housing stability that impact these health outcomes.
    • Have built and sustained partnerships in the communities they serve. 
    • Have demonstrated a strong track record of impactful community health, especially those who work directly with patients or community members.
  2. Mass General Brigham-based employees who:
    • Focus on one more of the following priority areas: Cardiometabolic Disease, Substance Use Disorder, Cancer, or Maternal Health
    • Exemplify an exceptional commitment to improving the health of communities.
    • Demonstrate a commitment to community health that goes above and beyond their job description (e.g. volunteerism or other depictions of community health service and leadership)

The awarded community organizations and MGB employee will be invited to attend an event on August 5, 2025 that will recognize their accomplishments. Awardees may also be asked to prepare a short presentation. More details to come.

Nomination Process

  • Nominations will be accepted only from MGB employees.
  • Self-nominations will not be accepted.
  • Nominators will submit a short application to nominate outstanding MA and NH-based community health organizations or an MGB employee.
  • Nominators will complete the brief application and provide information on the organization or employee their dedication to the community health field, their partnership/relationship with the organization or employee, and why they should be considered for the award.
  • Nominators will be given the option to include one or two additional recommendations from other colleagues or those impacted by their work (i.e. patient, community member, client).
  • Nominators may be asked to provide additional information during the review process.
  • Nominations are due through Survey Monkey Apply by Wednesday, July 2, 2025.

Evaluation of Applications

All applications will be evaluated for eligibility, and a committee will select the awardees. The committee will focus on the following criteria: 

  • Alignment to priority areas
  • Compelling description provided by nominator
  • Demonstrated commitment to improving the health populations in MA
  • Demonstrated community partnerships
